This is the checklist, in the order the documents are actually collected, with the recurring items marked. A driver qualification file is not a hiring folder that gets closed: four of the items on it recur on their own cycles and one of them can expire within three months of being issued. Everything here comes from the federal requirement rather than from a vendor's template.
Collected at hire
The driver's application for employment in the prescribed form. The road test certificate, or the licence the carrier accepted in place of a road test. A motor vehicle record from every state where the driver held a licence during the preceding three years, obtained within the required window of hiring. The medical examiner certificate together with verification that the examiner is listed on the National Registry. And the pre-employment Drug and Alcohol Clearinghouse full query, with the driver's specific electronic consent given inside the Clearinghouse itself.
Investigated at hire, and often the slow part
The safety performance history investigation: contacting employers the driver worked for in the preceding three years about accidents and about drug and alcohol programme violations. The carrier must make the request, document it, and keep the response or the record of the attempt where nobody replied. This is routinely the item that holds a file incomplete, because it depends on other carriers responding, and it is also the item most often confused with a DAC report, which does not satisfy it.
What recurs, and how often
The annual review of driving record, with a new motor vehicle record and the reviewer's dated certification of the review. The driver's own annual list of violations. The annual Clearinghouse query, which may be a limited query unless it indicates information exists. And the medical examiner certificate, which expires on its own schedule: two years at most, and frequently one year, six months or three months where the examiner wants to monitor a condition such as blood pressure. Those four are why file management is a continuing service rather than an onboarding task.
The eye chart, and what the physical is checking
The vision standard is one of the specific physical qualification requirements the medical examiner assesses, along with hearing, blood pressure and the review of medical history, using the standard chart to test distant visual acuity and also checking field of vision and the ability to recognise traffic signal colours. A driver who does not meet the standard may in some circumstances be eligible for an exemption programme. It is worth knowing that the examination checks defined standards rather than general health, because that is what makes the outcome predictable.
